Comparative Analysis of 6 Prognostic Scores for Ischemic Stroke


Background : Early prediction of functional outcome after ischemic stroke supports clinical decision-making and resource planning. Multiple bedside prognostic tools exist. This study assessed 6 established tools. Objective: To compare discrimination, calibration, and overall performance of the 6 tools for acute stroke outcomes in an observational cohort. Design: Prospective, multicenter observational study using the Fukuoka Stroke Registry across 7 tertiary hospitals (2007 to 2017). Participants/Methods: Among 12,486 patients with imaging-confirmed acute ischemic stroke, 12,015 had complete predictor and 3-month outcome data. Mean age was 72.5 years; 59.5% were male; 34.9% had poor outcomes. Patients with poor outcomes were older and had more comorbidities, worse pre-stroke function, more severe neurologic deficits, and more cardioembolic strokes.
